You will be responsible for ensuring that care is coordinated and that any issues relating to palliative and end-of-life care are raised. You will also be providing joint education, sharing best practice and ensuring communication to enable a seamless service for patient care.

As a senior member of the team you will act as a main point of contact and provide specialist symptom-control advice and on-going support to patients with specialist palliative care needs (as defined in the NICE Palliative & Supportive Care Guidelines) while supporting the use of end-of-life care documentation in the community

You will need excellent communication skills and the confidence to educate and engage with all staff groups, and should demonstrate the ability to work both in a team and independently.
